Custom Recruiting Event Request Form

Please complete and submit the form below at least two weeks prior to the event dates you would like. There is a small fee for creating a custom event with us: $25/visit for nonprofit companies and $50/visit for for-profit organizations, Payment is due no later than the first day of the event/visit. There are two ways to pay:

  1. Mail a check to Career Services, Azusa Pacific University, PO Box 7000, Azusa, CA 91702-7000. Please make the check payable to “Azusa Pacific University.”
  2. Pay with a credit card (Visa or Master Card) over the phone by calling (626) 815-2103.
