One Card Office

The One Card Office creates, issues, and manages APU student, faculty, and staff identification cards (ID). It is located in Cougars' Den Café and is open from 8 a.m. to 5 p.m., Monday through Friday.

Undergraduate students should read and familiarize themselves with the ID card policies below. Students are responsible for understanding and adhering to these policies.

Undergraduate Student Identification Card

The undergraduate student ID card is for your use as long as you are a matriculated student in good standing at Azusa Pacific University. It is to be used for dining, Cougar Bucks, library usage, check cashing, bookstore, fitness center, making payments at the Cashier's Office, school activities, and may be required by Campus Safety officers. Watch for new services in the future.

The ID card and all associated forms, records, and transcripts are the property of Azusa Pacific University. Use of the card may be revoked at any time. The cardholder agrees to abide by all rules, regulations, policies, and procedures specified by Azusa Pacific University. Violations of such rules, regulations, policies, and/or procedures may result in cancellation of a cardholder's account and/or a conduct report being filed. Future changes in the terms, conditions, and regulation use of this card will apply to all cards in circulation and use at the time, and will supersede the terms and conditions in effect at the time of this document.

Any Cougar Bucks balance remaining on the ID card upon graduation, formal withdrawal, or dismissal from the university will be refunded after a Cougar Bucks Refund Request is received. Cougar Bucks Refund Request forms are available at the Office of Student Financial Services. Evidence of graduation or formal withdrawal from the university must be submitted with the Refund Request. Refund checks will be mailed within 30 days after receiving such request for refunds of $100 or more, and will be sent to the address provided on the Refund Request form. Refunds of $100 or less will be refunded in cash at the Cashier's Office on East Campus. Any requests for refund of balance will be first applied to any individual debts owed to the university. In the absence of a Refund Request, accounts will be closed automatically three years after you have left the university through graduation, formal withdrawal, or dismissal. Accounts closed in this manner will have funds forfeited to the state of California.

Lost or Damaged ID Card Procedures

Please do not punch holes in ID cards. If a hole is found in your card, you will need to replace it. ID card holders can be purchased at the University Bookstore.

It is imperative that you report your card lost once you realize it is missing. During business hours, report the card lost to the One Card Office. During non-business hours, report the card lost to any dining location cashier. The missing card will be voided. You will be issued a temporary ID card for a nonrefundable $6 fee, which is good for up to three business days. All temporary ID cards must be returned to the One Card Office for your account to be activated. If you do not find your card, a charge of $20 will be accessed for a new card. If you return the temporary ID card, the charge for a new card is $14. Payment is required before the new card is made. If you received a receipt for a temporary ID card, please bring that along with the temporary ID card to the One Card Office.
