Use this to inform the Office of Student Employment when a student employee has ended his/her assignment with your department. This is an e-signature form and upon completion will automatically be submitted to the Office of Student Employment.
This form is to be used to request an adjustment in pay for a student employee who has either (1) demonstrated excellent work ethic and has worked two consecutive semesters in the same position, OR (2) has been moved into a lead role where their responsibilities have increased. Please review the student employee handbook for further instructions on this process.
Please use the Job Description Template (PDF) to upload required job description when selecting Option B on the Pay Adjustment Form in Formstack.
Use this form to request a change to the student employee’s current position (i.e., budget change, new supervisor, or new timekeeper). This is an e-signature form and upon completion will automatically be submitted to the Office of Student Employment.
The requisition form serves as a request for the Office of Student Employment to initiate the appropriate hiring procedures; the student is not officially hired until the Office of Student Employment has completed all hire documentation, and the student has been entered into the university’s automated timekeeping system.
