Graduate Student ID Card Procedures
How to Request a Graduate Student ID Card
Graduate student ID cards can be requested in the following ways:
- Email your request to onecard@apu.edu and include:
- Name
- Student ID number
- Home Address
- Indicate new or replacement ID card. (There is a $20 replacement fee.)
- Indicate non-photo ID or photo ID. (Photos can be sent electronically via email to onecard@apu.edu.)
- Fax a Graduate ID Card Request Form (PDF)* to the One Card Office in Hospitality Services at (626) 812-3057.
- Complete a Graduate ID Card Request Form in person at the Graduate Center, any regional center, or at the One Card Office in Hospitality Services.
- Download a Graduate ID
Card Request Form (PDF)* and mail to:

Azusa, CA 91702 - Send a letter of request to the One Card Office in Hospitality Services
and include:
- Name
- Student ID number
- Home Address
- Indicate new or replacement ID card. (There is a $20 replacement fee.)
- Indicate non-photo ID or photo ID. (Photos can be sent electronically via email to onecard@apu.edu.)
ID card requests are forwarded to the One Card Office for card production. The card is then mailed to the student’s home address.
Please allow 10 business days for processing of all ID card requests.

New ID Cards for Graduate Students
All graduate students are allowed one ID card while they are enrolled in classes at Azusa Pacific University. There is no fee for new ID Cards.
ID Card Pictures
ID card pictures are optional for graduate students. Pictures can be taken at Cougar’s Den Café during normal business hours (Monday through Friday 8 a.m.-5 p.m.). Pictures can also be sent electronically via email to onecard@apu.edu.
Replacement ID Card for Graduate Students
To request a replacement ID card, submit an ID card request form using one of the above methods. A fee of $20 is required for any replacement. Accepted payment methods are cash (in person only), check, or credit card (Visa, MasterCard, and Discover).
Temporary ID Card
A temporary ID card can be issued by the Graduate Center in cases where there is an immediate need. Temporary ID cards will be valid for only one week.
