Please bring the completed form with you for your consultation.

Malaria Precautions
If you are traveling to areas with risk for malaria, please review the Preventive Medications for Malaria document. Consult the Center for Disease Control (CDC) website for specific recommendations by county regarding which groups may need malaria prevention medications.

For additional protection against malaria-carrying mosquitoes, refer to the Technique for Impregnating Clothing or Mosquito Netting with Permethrin Solution document. This will explain how to use an insecticide solution for 1–2 sets of clothing or mosquito netting. This medication stays on the clothing for several weeks despite washing to deter mosquitoes. The solution can be purchased at outdoor sport shops like REI. No prescription is required.
